I got confused about gender several times (though the perm/lipstick/etc definitely leads to a hinting of the latter). The voice just sounds very female to me. Maybe emphasize the lack of athletics more, and less of the appearance aspects? Aunts crowding around and discussing a girl's physical and mental health sounds more natural than a boy's, but I could well be wrong.
